太監原來只是宮廷中的服侍人員，本無品級，隨著宮廷中太監人數的增加，分工的劃分，逐漸產生了等級。至雍正四年（1726年）形成定制。最大的官職為“督領侍”，相當於朝廷的正二品官銜。

不同品級的太監，穿著也不相同，一般來說，賞有太監頂戴的稱為老爺，無頂戴的稱為師父。在清末，督領侍張德安，長春宮的大總管張謙和，二總管阮進壽就是二品頂戴--“紅頂子”。
