Do I get to pick the name of my country and my title? Or do I somehow earn my title?
Are we allies? Can I start a war because why not? And who are my subjects? Can I marry into other kingdoms for democracy? *herald cries* "DEMOCRACY!"

You start with naming your kingdom on your own, unless you want to be something like a duke, which is under a king. Yes, you can have allies and enemies (not ACTUALL enemies). And yeah, it could be fun to get "married".
